Task: Split setup page into a simple 2 step process

The setup page should show a welcome message with some options for how to set up the application.
The standard/recommended option uses the existing setup form
The quick setup option uses the QuickSetup.aspx page
The restore option allows the user to either select a folder, or to upload a zip file containing the data to restore into the application upon setup
